MAIN SPONSOR SYMPOSIUM: From secondary care to the community, a joint approach on the quitters journey

        Presenter(s)

Wendy Preston  Nurse Consultant, George Eliot Hospital

Darush Attar-Zadeh  National Stop Smoking Trainer, Community Pharmacist & Respiratory Clinical Lead for Barnet CCG

        Abstract

Smokers may make numerous unsuccessful or partially successful quit attempts before finally managing to become smoke-free. How can we encourage more people to try to quit more often and to use the resources available to them to maximise their chances? Are we ensuring that all patients that begin their quit journey in Secondary Care are completing their journey in the Community?

Join Wendy Preston and Darush Attar-Zadeh as they use case studies to identify where joined up working between Secondary Care and the Community has yielded positive results, and what we can learn from mistakes made in the past.

Source of funding: This symposium is sponsored by GlaxoSmithKline Consumer Healthcare
